Dog Adoption Form

Before you begin this application, please ensure everyone in your family is ready for a new animal companion. Our animals are ready and waiting, but they deserve a loving home where they will be cared for and cherished as a member of the family.

We don't hold animals, so if you are planning a personal or work trip, please consider if this will hinder your availability to adopt a new pet.

If you aren't ready yet, it's okay; we will be here when you are ready.

Completing the application truthfully and accurately will expedite the review process. We try to respond to applications within 2-4 business days. Please make sure to "safelist" spca@triadspca.org so you don't miss correspondence regarding your application. We appreciate your patience. Because the SPCA of the Triad is trying to find the best match for our animals, we reserve the right to turn down any potential applicant.

You must answer all the questions. If any question doesn't pertain to you, please write "NA."

We require animals in your home to be current on their rabies vaccine, flea, tick, heartworm prevention, and spayed or neutered.

Our adoption fee for dogs is $300 for any age..

If you have a pet deposit, please list the amount and if it needs to be paid in full before the animal is adopted or monthly upon adoption.

Please be aware we require a pet deposit payment receipt before you take the animal home if your rental policy states a deposit is required.

Most of our dogs are not potty- trained. Are you willing to take whatever amount of time and steps are necessary to potty-train your new furry family member?

Do you understand it can take a while (weeks to months) for the animal to become adjusted and integrated into your family?

If your new family member develops any behavioral issues, how do you plan to handle the situation? Examples are excessive barking, resource guarding, potty accidents.

Do you understand animals need regular veterinary care such as annual shots, monthly preventives, dental, bloodwork, etc?

What do you feel as a valid reason to give an animal away?

If you are no longer able to take care of the animal, who will become their caregiver? This can include a temporary emergency, unexpected change in housing, or death.
